Overview
Creatinine is a byproduct of creatine phosphate metabolism in skeletal muscle and is excreted primarily through the kidneys. In healthy individuals, its production and excretion remain relatively constant.
Urinary creatinine measurement is commonly performed alongside other tests to evaluate kidney function and assist in the diagnosis and monitoring of renal impairment. A random (“spot”) urine creatinine level is frequently used in combination with serum creatinine, serum sodium, and urine sodium to calculate the fractional excretion of sodium (FENa). FENa reflects the proportion of filtered sodium that is excreted in the urine versus reabsorbed by the kidneys.
Clinically, FENa helps differentiate the underlying causes of renal dysfunction, whether prerenal (e.g., hypovolemia), intrinsic renal, or postrenal (e.g., obstruction).
